What should we do when a student skips lines in a text and doesn't realize they are skipping? Do we focus on self monitoring and/or something else?

Is this happening as you listen in during a lesson? You could stop the child at the end of the page and ask if it makes sense, use prompting language for SM, searching for M, something didn't quite match when you were reading, etc. The prompts would depend on the level of the reader and the strategic action you think is needed.
If you know the child has a vision problem you can allow a finger at the end of the line, a bookmark or an accommodation as the font size becomes smaller or lines increase with less spacing. Do not let the reading become dysfluent even if an accommodation is used.
