Student read sentence; however, added a whole new sentence, e.g.,
"Nick put on his pajamas." added "he went to the washroom".

Do I count each word from the new sentence as an error?

Yes. Each error is counted and scored as an error as described on the Coding and Scoring Errors at a Glance. Please note the Special Cases for Assessing Accuracy on page 23 of the BAS 1 3ed Assessment Guide about students inserting more words than are on the page or are inventing text.

Best wishes!
